The strategy is focused on empowering people and giving them the resources and support to not only deal with their issues but thrive thereafter. What is Integrated Care?

Integrated care is a holistic approach to treatment that offers specific services based on the full range of a person’s needs. Rather than focusing solely on the presenting issue of alcohol or drug use, integrated care focuses on mental health, physical health, and social well-being.

Many people who seek support for alcohol and other drug (AOD) concerns also experience challenges related to mental health, housing, employment, relationships, physical health, or legal matters. Our support services aim to address the complex needs of our clients by empowering individuals to make informed choices and take active steps in their own recovery journey.

The Integrated Care Model

The Integrated Care model adopts a holistic approach that coordinates treatment across the different specialist areas. The more complex the conditions of our clients, the more integrated the support will be.

Trauma-informed Therapy

Trauma-informed therapy is our approach to treatment that involves understanding, recognising and responding to the effects of trauma. Trauma-informed therapy at Holyoake is based on the principles of:

What is Case Management?

Case management is essential to integrated care, offering a collaborative and supportive approach to ensure our clients are connected with the services they need. Its goal is to offer whole-person support, ensuring the best possible outcomes for every client.

The case management model recognises that alcohol and other drug issues do not exist in isolation from other health and social issues. Case management links clients with appropriate services to address their needs and achieve their goals.

Failure to address co-occurring issues may exacerbate alcohol and other drug use and result in treatment drop-out. There is evidence that the longer you stay in treatment, the greater the chance of a better outcome.

Our Case Management Process

The case management process is designed to guide our patients through their recovery journey by providing them with tailored support based on their needs and preferences. Here is what the process typically looks like:

Initial Assessment:

The process begins with a comprehensive assessment of the client’s needs, challenges and goals. This helps us to understand our clients’ mental, social, physical, and AOD issues and design a plan to address them.

Connecting to Services:

Based on the initial assessment, both the client and the case manager will work together to build a personalised care plan to ensure the client can begin making progress. From there, our case management team will provide you with a point of contact with other services, including health and medical, mental health, housing, legal, employment and social services.

Ongoing Monitoring & Support:

Throughout the recovery process, the case manager will monitor the client’s progress, ensuring adjustments are made where needed. This ensures that the client is always receiving the most relevant and effective support.

Follow Up:

Once our clients have achieved their goals and feel confident in their progress, they may no longer require our services. However, follow-up support is always provided to ensure continued success.

What is the No-Wrong-Door Approach?

The principle that underpins integrated care and case management is individualised service delivery and the concept of ‘no wrong door’. Adopting a ‘no wrong door’ policy means that if you present to Holyoake, you will have access to the required services. This coordinated approach acknowledges that AOD issues do not sit in isolation and are often related to other health and social issues.
