AFTER-CARE CO-ORDINATOR

The After-care Coordinator provides a vital link between hospital and community-based services, via Country WA Primary Health Network.

Funded by WAPHA, this service is available for people experiencing suicidal thoughts without immediate intent and for people after a suicide attempt.

The service recognises continuity of care during the first few days after discharge from hospital following a suicide attempt. This is a known high-risk period for suicide and a period where people need support to link back in with their community and networks.

The After-care Coordinator helps manage risk, makes safety plans and supports people to connect with other services. The service also provides practical ways to reconnect people with their GP and other relevant community agencies.

Referrals for the After-care Coordinator come via the hospital, GP clinics, or from the Community Mental Health Service.
